| Drug hypersensitivity reactions may be immune or non-immune mediated. Immune mediated reactions may be further differentiated into innate and adaptive mechanisms. Non-immune mediated reactions are also known as pseudoallergic, anaphylactoid or infusion reactions. These reactions most often occur with the first dose and produce a variety of symptoms. These include mild rashes and fever to severe anaphylactic shock and respiratory distress. Some drugs such as morphine and vancomycin directly stimulate mast cells to release histamine. Others may be directly activate complement. Certain monoclonal antibodies may also trigger the release of pro-inflammatory cytokines from lymphocytes such as observed with the TeGenero anti-CD28 antibody. Various in vitro methods to predict these reactions during drug development are in different stages of development. |
